Support provided the following, not so helpful, reply and I am posting with permission from the person who contacted support:
You indicated the 1099 NEC form is not lining up correctly. You wrote "I have tried customizing the form to fix the alignment but when I get the 1st and 2nd one correct, the 3rd one goes to page 2."
So are you saying the first two forms are lined up now? What is off on the third form? Is it too high, too low? By how much?
There are two group footers on the vendor ID. Group Footer 2a prints the data on the form, and group footer 2b provides the appropriate spacing between the printing of the forms by group footer 2a.
If the first form is lined up correctly, and the second and third forms are a little bit too high, increasing the size of group footer 2b should force the second and third forms down a little bit.

All,
Thanks for the update of the FED table.
Now I have question/issue with the new 1099-NEC 3-UP format.
Has anyone gotten it to work correctly yet. I have been trying to adjust things in the format and have been able to the first two vendors to just about line up but the third vendor goes to the next page. I have tried margins, size of the group 2 footer, etc. but cannot get it to line up correctly.
I am open to any ideas.

Several issues going on here:
- The 12/31/2020 PR Tax tables were incorrect (they had 2020 data, not 2021) As per IRS Pub 15-T for 2021
- The 12/31/2021 PR Tax tables are incorrect (they have 2021 data, not 2022) As per IRS Pub 15-T for 2022
- Not only are FET1 (Single/MFS) and FET2 (MFJ) tables incorrect, but also FET3 (HH), and FET6, FET7 & FET8 (for when Step 2 of W-4 is not checked).
I modified the 030200FE.INT file and attached it here (had to rename it as .TXT). Also attaching the relevant page from the 2022 Publication 15-T which has all the withholding rate tables.
You can use the 030200FE.TXT file (download and rename as 030200FE.INT) and import this instead (or over top) of the file that came with the 123121 PR Tax update.

FET1 and FET2 are definitely not the same from last year, see highlighted difference below from 2020 on the left to 2021 on the right. However the IRS publication does not match the tables either so hopefully there will be an update.

To all,
An FYI in regards to 2022 Tax Updates.
The FET1 and FET2 tables are incorrect again. They show 2021 values.
Is supposed to be corrected for 1/31/2022 tax update.

The SL 2018 CU7 was available on Friday but gave an error on download. Today I was able to download SL 2018 CU7.
The 2022 Tax Updates are not yet available.
Hope you find this helpful.
